Gossoms End Surgery

Patients have the following options as to how they may request repeat prescriptions:

1. By taking their repeat prescription request form to the chemist of their choice in Berkhamsted.
2. By bringing their repeat prescription request form to the surgery. A box is provided in the entrance hall, or request forms may be left with the Receptionist.

4. By fax to the surgery.
5. By post to the surgery (include a stamped self-addressed envelope).
